What is performed?
- Examinations are performed in accordance with DOT regulations and include:
- Health History/review with provider
- Vision Testing
- Hearing
- Vital signs (blood pressure, pulse, height and weight)
- Laboratory testing
- Protein, blood and sugar levels
- Evidence of diabetes or renal impairment
- General Physical exam
- Determines if the body is ‘abnormal’ in any way that can prohibit a job function
- Skin, cardiovascular, back/spine, hernia, abdomen, etc.
What is provided?
- General certification of pass/fail
- Full report upon request
- Driver Card made available upon request to client
- Employee will receive at clinic

What is performed?
- Mirror of the Federally Regulated DOT Physical:
- Health History/review with provider
- Vision Testing
- Hearing
- Vital signs (blood pressure, pulse, height and weight)
- Laboratory testing
- Protein, blood and sugar levels
- Evidence of diabetes or renal impairment
- General Physical exam
- Determines if the body is ‘abnormal’ in any way that can prohibit a job function
- Skin, cardiovascular, back/spine, hernia, abdomen, etc.

Lift Tests
- Standalone order
- 50 or 75 lbs
- Evaluates an individual’s ability to safely lift a given weight
Biometric Screen
- Height, Weight, blood pressure, BMI, lipid plus glucose
- Online Scheduling and reporting
Tuberculosis Testing
- TB Skin - Step 1
- Determines if the individual has developed an immune response the bacterium that causes TB
- If positive or if the person has had a positive TB test in the past, a chest X-ray may be indicated to determine whether the person has active or latent TB
- TB Skin - Step 2
- Used to detect individuals with past TB infection who now have diminished skin test reactivity. It requires placement/reading of two separate skin tests

- Projection radiograph of the chest used to diagnose conditions affecting the chest, its contents and nearby structures

What is a Titers Test?
A blood test that checks an individual's immune status to vaccines or a disease they may have come in contact with in the past

Vision/Audiograms
- Vision Test Ishihara
- Color blindness test for Red and Green deficiencies
- Examiner will hold up a colored plate that contains a number made of dots
- Those who are affected by color blindness, will have a difficulties seeing the number
- Vision Test Snellen
- Eye chart used by eye care professionals
- Contains 11 lines of block letters, progressively going from large to small
- Vision Test Jaeger
- Using printed letters of different sizes that test the acuity of near vision
- Vision Test Titmus
- Tests for acuity (near: 14 inch, far: 20 ft),
- Depth/color perception
- Muscle balance (lateral and vertical phoria)
- Horizontal visual fields (peripheral vision of 130° in each eye)
- Performed using a single lightweight, compact, and easy-to-use instrument
- Audiogram
- A way to measure a person’s hearing loss
- Range from 100 Hz to 6,000 Hz
- Produced using a piece of test equipment called an audiometer, which allows different frequencies to be presented to the subject
- Calibrated headphones are used at various levels
Vaccines
- Varicella Vaccine
- Protects against the viral disease commonly known as chickenpox, Herpes zoster, and Postherpetic neuralgia
- TDAP Vaccine
- Protects against three potentially life-threatening bacterial diseases: Tetanus, Diphtheria, and Pertussis (whooping cough)
- Hepatitis B Vaccine
- Protects against the Hepatitis B virus.
- This immunization is administered in a series of 3 vaccines
- Second dose given 4 weeks after the first, and the third dose 5 months after the second
- Meningococcal Vaccination
- Can help prevent meningococcal disease, which is any type of illness caused by Neisseria Meningitidis bacteria
- MMR Vaccine
- Protects against the measles, mumps and rubella
Other Occupational Health Services
- OSHA Respirator Questionnaire
- When OSHA requires a respirator, the employee must fill out a questionnaire
- Determines if the employee is physically/psychologically able to wear a respirator
- Must be reviewed by a physician or licensed healthcare professional
- Based on response, the healthcare professional could require additional testing
- Physical, X-Ray, etc

- Pulmonary Function/Spirometry
- Measures how well the lungs take in and release air
- Respirator Fit Test
- Qualitative
- Pass/fail to confirm if a respirator masks fits correctly
- Taste/smell are used to test
- Individuals should bring in their own mask as the ones the clinic may have are different that what will be used in the workplace
- Quantitative
- Measures the ‘leakage’ into the respirator using an agent
- Capable of measuring fit factors from 100 - 10,000+
- Qualitative
