When utilizing the Applicant Invite option, candidates are sent a customizable email with a link to digitally sign the required Disclosure and Authorization forms, and fill in any additional personal identifying information necessary to complete the screens you requested:
When the candidate follows the link in the email, they are first presented with a table of contents outlining what to expect in the questionnaire:
Candidates will click "Get Started" and will be presented with the Electronic Signature Consent that they will need to read and sign at the bottom of the screen:
They will check of the box indicating that they understand what they are signing and will either type-to-sign their acknowledgment or can use a mouse or finger to sign, if on a mobile device. The candidate will also need to enter the last 4 digits of their social security number or user ID, if they don't have a social security number:
If the user clicks the "I do not wish to electronically sign my documents" box, the order will be cancelled and you will be notified by email that the candidate declined the invite.
The user will then click "Next" and will be provided an opportunity to update the state of employment as well as their state of residence:
The next screen presents the disclosures appropriate to the candidate's location, the work location, the employer's office location, products being ordered, etc. that the user must acknowledge before they can proceed:
Another example of a disclosure form that likely would be shown to the candidate is the summary of rights under the Fair Credit Reporting Act (FCRA):
Once the user signs/acknowledges all of the required forms and clicks "Next," they will then be presented with the personal information screen where they will in put their data (items denoted by an * are required fields):
Followed by their current address and any previous addresses:
Finally, before submitting the application, the candidate is presented with an application summary page, which gives them the opportunity to correct any data they may have entered incorrectly:
Once the candidate clicks "Submit," the order is then placed and Verified First begins processing the screens, or if you utilize manual review ordering you will be notified to review and order.
